> Ask for the year-end consolidated Form 1099 from each brokerage account, which combines the 1099-B, 1099-DIV and 1099-INT, rather than monthly statements. Ask the client to list every account so none is missed, and to wait for any corrected version before sending if the broker has announced one. Cost basis on each sale is what you check first.

## Why tax preparers need an investment statement

The consolidated 1099 is what the broker reported to the IRS: dividends and interest flow to Schedule B and Form 1040, and each sale goes onto Form 8949 and Schedule D. Monthly statements show balances, not the tax figures, and their totals rarely match the 1099. Where basis was not reported to the IRS, you need the client’s own records of what they paid, or the gain is overstated.

## What exactly to ask for

The final consolidated 1099 for the tax year from every taxable brokerage or investment account, all pages, including the supplemental information section. If a sale shows “basis not reported to the IRS”, also ask for the purchase confirmation or the account’s cost basis report. Retirement accounts send different forms and are a separate request.

## What to check when it arrives

- It is the final or corrected 1099 for the right tax year, and the account number matches the client’s list.
- Every sale has a cost basis, and sales marked noncovered have the client’s purchase records.
- Wash sale adjustments and foreign tax paid are noted, since they change the figures you enter.

## Email to send your client

Subject: Your brokerage 1099s for the year

```text
Hi [Client name],
For your investments I need the year-end tax form from each brokerage account, usually called a consolidated 1099 or tax reporting statement. Your monthly statements will not work for this; the tax form has the figures the IRS receives.
You will find it in the tax documents section of each account online. Please send all pages, and tell me if the broker says a corrected version is coming.
Could you send them by [date], with a quick list of all your accounts? If you sold something bought long ago and the form shows no purchase price, let me know and we will find it together.
Thank you,
[Your name]
## Ask once. DokuTrak does the chasing.
- Your client uploads from one link, with no account and no password.
- Reminders go out on a schedule, by default 2, 5, 10 and 14 days after the request, until the file arrives.
- With AI review switched on in your workspace (it is off by default), each upload gets a first pass that flags a file that is not an investment statement or an unreadable or blurry scan. It says so when it is not sure.
- You accept every file or mark it for re-upload, and your client gets the note by email. The AI never accepts a file.
